Our Services

Comprehensive care solutions tailored to your unique needs, designed to help you maintain independence and quality of life.

    Home and Community-Based Services

HCBS (Home and Community-Based Services) are Medicaid-funded programs that let adults receive support in their own home or community instead of a facility. Minnesota offers several HCBS waiver programs — including CADI, DD, BI, and CAC — each designed for different needs. As a 245D licensed provider, Midway Homes delivers services under these waivers.

CADI Waiver — Community Access for Disability Inclusion

    DD Waiver — Developmental Disabilities

    BI Waiver — Brain Injury

CAC Waiver — Community Alternative Care

    Elderly Waiver

    Basic Support Services

Day-to-day assistance that helps adults live safely and independently at home. Basic Support includes homemaker services (cooking, cleaning, laundry, errands), night supervision, 24-hour emergency assistance, adult companion services, individual community living support (CLS), individualized home supports, and respite care.

    In-Home Support Services

Individualized Home Supports (IHS) delivered in your own home. IHS with training teaches daily living skills — meal prep, budgeting, hygiene, household routines — so you build independence over time. IHS without training provides direct hands-on help with those same tasks. Family training helps family members learn techniques to support their loved one.

    Integrated Community Supports (ICS)

ICS is 24-hour staffed supportive housing in a provider-managed home for adults 18 and older on a BI, CAC, CADI, or DD waiver. Residents have their own space with on-site staff who help with community participation, health and safety, household management, and adaptive skills — the goal is building independence while living in a supportive setting.

    Personal Care (PCA / CFSS)

Personal care support in your own home. Minnesota is replacing Personal Care Assistance (PCA) with Community First Services and Supports (CFSS). Midway Homes helps with activities of daily living such as bathing, dressing, grooming, toileting, mobility, and eating, so adults can stay in their own homes and communities.

What Do 245D, HCBS, and ICS Mean?

245D — Minnesota's Quality Standard for Home Care

245D is a Minnesota state license that sets standards for agencies providing home and community-based services (HCBS) to people with disabilities. A 245D license means the agency meets DHS requirements for staff training, background checks, person-centered service planning, incident reporting, and quality assurance. When you choose a 245D licensed provider, you know the state has verified the agency meets these standards.

HCBS — Home and Community-Based Services

HCBS stands for Home and Community-Based Services — Medicaid-funded programs that allow adults to receive care in their own home or community instead of a nursing facility or institution. Minnesota offers several HCBS waiver programs (CADI, DD, BI, CAC, Elderly Waiver) that fund services like homemaker, in-home support, community living support, and ICS. Your county case manager determines which waiver you qualify for based on your needs.

    ICS — Integrated Community Supports

ICS is a specific HCBS service where you live in a staffed, provider-managed home — not a facility, but a regular house with 24-hour support staff on-site. You have your own private space, and staff help you build skills in four areas: community participation, health and safety, household management, and adaptive skills. ICS is available to adults 18 and older on a BI, CAC, CADI, or DD waiver.
